How to Refer a Participant to Enhance Nursing Care and Support Services
At Enhance Nursing Care and Support Services, we make the referral process simple, transparent, and participant-focused. We work closely with participants, families, careers, and support coordinators to ensure that each individual receives the right support, at the right time, tailored to their unique needs.
Who Can Make a Referral
Referrals to Enhance Nursing Care and Support Services can be made by:
- NDIS Support Coordinators
- Family Members or Carers
- Participants themselves
- Allied Health Professionals (such as Occupational Therapists or Social Workers)
- Hospital discharge planners and case managers
- NDIS Local Area Coordinators (LACs)
We welcome all referrals and are committed to responding promptly and professionally.
The Referral Process

Initial Consultation
We will arrange a consultation to understand the participant’s goals, preferences, and care requirements. This may be conducted in person, over the phone, or via video call.

Assessment & Support Planning
Our team will conduct a comprehensive assessment and work with the participant and their support network to develop a personalised support plan.

Service Agreement & Implementation
Once the support plan is agreed upon, we create a clear service agreement and begin delivering tailored supports in a safe, professional, and compassionate manner.

Ongoing Review & Communication
We continuously monitor and review supports to ensure they meet participant goals, adapting services as needs change. We maintain open communication with participants, families, and support coordinators at all times.
